  • Opal Therapy for Healing is in-network with certain Blue Cross Blue Shield PPO plans including the following.

    BCS - Professional Blue Choice PPO

    PPO - Preferred Provider Organization

    All clients are encourage to confirm that Opal Therapy for Healing is in-network with your insurance provider by calling the number on the back of your card.

  • How much is my copay per therapy session?

    What is my in-network deductible for outpatient mental health services?

    If I have a deductible, how much is it? Once it’s been met, what percentage of sessions are covered by my plan?

  • If your insurance is out-of-network, you will be responsible for the full payment at the time of service. A superbill (proof of payment) can be provided for you to submit to your insurance company. Some plans will reimburse a portion or all of session costs.

    To understand your benefits, please call the number on the back of your insurance card to ask about out-of-network mental health coverage for CPT codes 90791, 90837 and 90834.

    Opal Therapy's out-of-network rate is $185 per session. The first 3 sessions typically use CPT code 90791 (psychiatric diagnostic evaluation) and the following sessions use CPT code 90837 or 90834.

  • Do I need a referral from my primary care physician to see a mental health professional?

    Do I need pre-approval from the insurance company before seeing a mental health professional?

    What is my out-of-network deductible for outpatient mental health visits and must this be met before receiving reimbursement?

    How do I submit claims for out-of-network reimbursement?

  • Payment is due at the time of service.

    We accept credit card and Zelle.

    A valid credit card must be kept on file through your SimplePractice portal, regardless of how you plan to pay. Services may be suspended if the card on file becomes invalid and/or payments don't process.

    When paying by credit card, there is an additional 3% processing fee. There is no fee when using Zelle.

  • Scheduled sessions are reserved exclusively for you, therefore a 24-hour notice is required for all cancellations and requests to reschedule.

    Clients who do not show or cancel late will be charged the out of pocket rate of $185 whether or not your insurance is in-network. Insurance does not cover no show/late cancellation fees.
